"When All else Fails - If you've read my first procedure review, you know what's happening. If not, I'll give a brief synopsis for you.Back in 2016; I worked my a** off, literally. After losing 90 pounds I was left with stubborn belly fat and saggy skin around my abdomen. It was really affecting me and my view of my body. I worked extremely hard and felt I had nothing to show for it. Enter in, Dr. Dorner.I had my tummy tuck with muscle repair and liposuction done in September of 2016. 12 weeks into recovery..........Pregnant.I was extremely worried. I had just had this major abdominal surgery and now it was going to be undone; would it be okay for the baby, is it going to be too tight, would there be any complications? I had an amazing OB/GYN and it turned out her brother is a plastic surgeon. She said that she has had several patients that have had a tummy tuck and then had babies, with little complications, although they hadn't been as soon after.Long story short, everything turned out fine. I was fine, baby was fine (although she was breech and we couldn't get her to turn - no matter how hard we tried) and she was born in August of 2017.During my pregnancy I gained about 29 pounds. After my pregnancy, I gained additional weight. You know....new baby, whatever sleep I could get, nursing..these things took priority. I have managed to lose 23 pounds. So that's great, but it hasn't been lost from where I want. I've been trying hard, not as hard as before but still hard and the fats just not budging.I saw Dr. Dorner today for a consultation; after looking at the areas around my hips, he agreed and said he could perform liposuction with no issues. He did ask if I had had any drastic weight loss prior to my tummy tuck because I have more skin than he'd expect for someone who gained a small amount during pregnancy. I can't fault him for asking, I doubt he remembers my previous history, so I said yes. He briefly mentioned a posterior body tuck which I guess is like a tummy tuck for the backside. No thanks. Types of Facial Implants
A straight, strong jawline is a universal sign of beauty throughout all cultures. As a person ages, the outer jaw starts to lose width and the straight line begins to soften. Jaw implants provide improved outer jaw and angle of the jaw definition. The type of jaw shape can also be altered, from a heart shape for a more feminine appearance or a squarer jaw to increase masculinity.
The procedure is performed in the surgery center under general anesthesia. The incisions are placed within the mouth, so they are not visible. The implants are pressed against the bone and held in position with small screws. The operation takes about an hour and patients return home the same day. Most patients will be to return to work within a week. Some bruising and swelling is to be expected but resolves within 2 – 3 weeks.
Tear Trough Implants
Tear trough implants are designed to fill the dark hollows under the eyes. Patients who have deep set eyes and indentations from birth or from aging do well with tear trough implants. These also have excellent effects when added to a lower eyelid lift (blepharoplasty).
The procedure is performed in the office or surgery center under IV sedation or general anesthesia. A small incision is made behind the eyelid, so they are not visible. The implant is placed directly on the bone and held in place either with sutures or screws. The operation takes less than an hour and patients go home the same day. Recovery is about a week until swelling has subsided. Bruising around the eye can be unpredictable, but it typically lasts between 1 – 2 weeks. When combined with a lower eyelid lift, recovery is generally about the same, taking about a week to return to work. Pain is not significantly increased when doing tightening at the same time.
Temporal augmentation has become very popular over the past several years. Fillers and fat have been used to augment the temporal region, but these are less precise and fillers are temporary. Temporal implants provide instant, permanent augmentation through a very easy surgery. Anyone who does not like the hollows within the temporal region is a candidate for this procedure. Some patients may even have hollowing due to medical conditions such as congenital growth issues or facial wasting from medications.
The procedure takes about thirty minutes to perform. It can be done in the office or surgery center under IV or general anesthesia. Patients go home the same day and recovery is minimal. Most people will return to work within one to two days. Various implant shapes are available to augment the lateral temporal zone. The incisions are hidden within the hair and heal very well.
Custom implants are a rapidly evolving field of facial implants. With the advent of 3D printers, the ability to create individualized implants has exploded. A CT scan of the patient is used to provide the basis for a computer modeled implant design. Plastic models of the implants can be printed and reviewed before the actual implant is created. This allows the surgeon and patient to review the intended augmentation and fine-tune any details. These implants are particularly useful for chin and jaw augmentation procedures, especially when the entire jaw is being lengthened. They are also particularly well suited for patients need improvement in the orbital rim, zygomatic bone, and lower mid-face. Anytime two or three implants are being considered, rather than just one, custom implants should be considered.
The procedure is the same as other implants being placed, however, these are designed to fit directly on the skeleton like a puzzle piece. Longer jaw implants are placed through two incisions – one within the mouth and the other the same for a chin implant. This allows the entire implant to be placed in a smooth continuous tunnel.
What to Expect with Facial Implants
Following the placement of a facial implant, you should expect to notice bruising, swelling, and some discomfort for around two weeks. You should also avoid strenuous exercise for around 7 – 10 days. Any additional aftercare instructions will be provided by a member of our experienced team. Most patients will be able to return to their normal daily activities within two months of their surgery with noticeable aesthetic results.

Can fat grafting be used to fill in facial features?
Dr. Dorner is skilled at offering facial fat grafting for patients who wish to use their own fat instead of an implant to add volume. Patients must have enough fat to use for the transfer in order to be a candidate for facial fat grafting.
